***Please Read All Instructions Prior To Completing the Pre Qualification Form***

The Pre Qualification Form utilizes Adobe Acrobat 9. If you have an earlier version of Adobe Acrobat you will be able to fill out the Pre Qualification Form and print it; however, you will not be able to save it. In order to save the document you will need Adobe Acrobat version 9 or later.  Click here to download a free version of Adobe Acrobat 9.
 

To Submit the Pre Qualification Form ONLINE:

        1.  After completing the form press the SAVE Button located at the bottom the form 
        2.   Save the document onto your computer 
        3.   Open your personal email account, create a new email and attach the saved document to the new email 
        4.   Send the email with the attached document to info@loanfund.org 

To Submit the Pre Qualification Form Through the MAIL:

        1.  After completing the form press the PRINT Button located at the bottom of the form 
        2.  Adjust the print options to your preferences and print the document 
        3.  Mail the completed Pre Qualification Form to:

Be aware that typing your name into the signature box on the last page of the form constitutes a legally binding signature. 
Print a copy of your completed form for your records.A Loan Officer will review your application and will contact you by telephone within five (5) business days of receiving your Pre-Qualification Form.

*Filling out a Pre-Qualification Form does not automatically qualify the applicant for a loan.

IF YOU DO NOT PRE-QUALIFY:

Your Loan Officer will discuss alternative options with you, including possible referrals to other organizations.

IF YOU ARE PRE-QUALIFIED:

As instructed by your Loan Officer, please fill out the Pre-Qualification Addendum Form here. You will also need to submit:

• Your business plan (if applicable).

• Copies of your personal tax returns for the past two (2) years.

• Copies of the business and tax returns for the past two (2) years.

• Collateral information

• A loan application fee ($25 for sole proprietors, $50 for corporations) made payable to The Loan Fund.
